Executive Summary

We are the communities we serve. Our community health worker (CHW) model fosters local, sustainable solutions that address the root causes of health disparities and ensures our programs are informed by our collective experience and cultural understanding of systemic and situational barriers to health equity. 90% of our staff and 80% of our board are first generation immigrants and ensure our work remains effective and community led. With 44 years of experience pioneering the CHW model in MN, we have deep roots in community health practices and long-established community trust. Our clients come to us as a safe harbor, often referred to by friends or family members. WellShare’s CHWS are a racial equity workforce that reaches socially isolated communities and disrupts a healthcare structure that prioritizes privilege at the expense of marginalized communities’ health. We work upstream to address systemic barriers and social determinants of health to disrupt the cycle of generational poverty, disease, and trauma. We support our clients' physical, mental, and financial fitness across 23 programs and six impact areas: maternal and child health promotion, youth health leadership, mental health education and outreach, chronic and infectious disease prevention and management, health system navigation and transformation, along with workforce and economic development. We are making a difference in ways that both improve and save lives of individuals, families, and their broader communities. Our staff, who often live alongside the people they serve, witness the direct impact of our services in real time.
